By checking this box, I ("Participant" or "Parent/Guardian") acknowledge and agree that participation in this class, event, service, or rental ("Activity") involves inherent risks, including but not limited to physical injury, illness, property damage, or other harm, whether caused by my actions, the actions of others, equipment, facilities, or environmental conditions.

I voluntarily assume all risks associated with participation in the Activity, whether known or unknown, foreseeable or unforeseeable. I represent that I am physically able to participate in the Activity and agree to follow all instructions, rules, and safety guidelines provided by the Host.

To the fullest extent permitted by law, I release, waive, and discharge the host of the Activity ("Host"), and its affiliates, event sponsors, and employees from any and all claims, demands, damages, injuries, losses, liabilities, or causes of action arising out of or related to my participation (or my minor child's participation), including claims based on negligence.

I assume responsibility for any damage caused by myself to the Host’s facilities or equipment, including but not limited to furniture, set pieces, lighting, cameras, etc. The Host agrees to provide me with a detailed statement of all such damage and cost of repair, normal wear and tear excepted, and such costs will be my responsibility to fulfill.

I acknowledge that Get Werk, Inc. ("Werk") is a technology platform that facilitates bookings only, is not the provider of the Activity, and is not responsible for the conduct of the Host or other participants. I release Werk from any claims arising out of or related to participation in the Activity.

By checking this box, I confirm that I am at least 18 years of age, or that I am the parent or legal guardian of a participant under 18, and that I agree to this Liability Waiver on their behalf.
